When booking, you will be asked to provide your name, date of birth, contact details, and whether you have visited the practice before. If you qualify for a free NHS-funded eye test, make sure to mention this — you may need to provide proof of eligibility (such as a benefit letter or proof of age) at your appointment. If you wear contact lenses, mention this when booking so the correct appointment type can be allocated.
